What if an element or a modality is missing from the chart?
Missing does not automatically mean broken. Looking at the distribution is useful mostly for seeing which energy is abundant in one particular chart and which is thin on the ground. Suppose someone has the Sun in Pisces, the Moon in Cancer and the ascendant in Scorpio. All three personal points land in water. Emotional sensitivity is likely to be pronounced for this person, while bringing ideas down into concrete form may take more work, precisely because earth is running thin in that map.
A weak element does not switch that area off altogether. It asks for more conscious effort, that is all. Someone short on earth can learn over the years to keep routines and to plan without it feeling like a foreign language. Excess produces its own imbalance too. A chart heavy on fire tends to arrive sooner or later at the need to learn how to brake, given that the natural impulse always runs towards speeding up.

Are elements and modalities calculated from the sun sign alone?
No, the full chart is needed. The Sun, the Moon, the ascendant and every other planet each contribute their own element and modality. Stopping at the sun sign gives an incomplete reading of that distribution.
Are mutable signs indecisive by nature?
It is closer to adaptability than indecision. Mutable signs adjust course easily and manage several things at the same time. That flexibility can look like a lack of focus, though it usually works as a strength.
Do three signs of the same element always get along?
They tend to start from comfortable ground, since the angle between them is 120 degrees, a trine. That favours a shared language and an easy flow of energy, although the rest of each person's chart also has a say.
